Does any of this
sound familiar?

Parents almost never arrive with a diagnosis. They arrive describing what they can see. Below is what those descriptions usually turn out to mean — and what can be done about each one.

None of this is a diagnosis. Every child is different, and the only way to know is an assessment.

Ankle foot orthosis · any age

My child's foot drags or turns in when they walk

An ankle foot orthosis holds the foot in position through each step, so the leg doesn't have to swing out to clear the ground. Common after a stroke, in cerebral palsy, and in children whose walking pattern hasn't settled.

Knee ankle foot orthosis · any age

My child's knee gives way under them

When the knee can't take weight on its own, a full-leg brace does that job. The joints either lock for stability or move freely, depending on how much control your child has.

Scoliosis brace · usually 8–16 years

Her back looks uneven when she bends forward

One shoulder sitting higher, a rib hump on one side, a waist that isn't symmetrical. If a specialist confirms a curve, a brace worn during the growing years is used to stop it progressing.

Cranial remoulding helmet · from 4 months

My baby's head looks flat on one side

Very common, and often correctable. If changing sleeping position hasn't helped by around four or five months, a custom helmet gently guides how the skull grows. It doesn't hurt, and babies usually stop noticing it within days.

Custom seating system · any age

My child can't sit up without being held

A moulded seating system supports the trunk and head so a child can sit safely — which usually means breathing and eating more easily, and being part of what's going on around them.

Insoles and foot orthoses · any age

My child says their feet hurt after walking

Custom insoles spread pressure across the foot and support the arch. Also used for diabetic feet, where protecting the skin matters more than comfort does.

Orthopaedic footwear · any age

Normal shoes just don't fit properly

Where a foot is a different shape, one leg is shorter than the other, or a brace has to fit inside the shoe, footwear can be made to measure rather than adapted.

Ear moulding · first weeks of life

My newborn's ear is folded over

Ear cartilage is soft in the first weeks after birth and can often be reshaped without surgery. This one is the most time-sensitive of everything here — ideally within the first three weeks.

Why timing matters

Some of this
has a window.

Not everything is urgent. But a few of these treatments work by guiding growth — which means they only work while that growth is still happening. If you're weighing up whether to wait and see, this is the part worth reading.

Ear moulding

First 3 weeks

Cartilage is at its softest immediately after birth. After about six weeks it stiffens, and non-surgical correction becomes much harder or stops being an option.

Cranial helmet

4 to 6 months

The skull grows fastest in the first year, and a helmet works by guiding that growth. Starting between four and six months is usual. It is rarely worth beginning after about fourteen months.

Scoliosis brace

While still growing

A brace can influence a curve only while the spine is still growing. Once growth is complete, bracing no longer changes the curve, though it may still be used for comfort.

Walking orthoses

As soon as you notice

No fixed window here. But the longer an unusual walking pattern goes unsupported, the more the hips, knees and back compensate for it.

If you're not sure whether it's too early or too late to ask — ask. Too early has never been a problem for anyone.
What actually happens

From the first visit
to the fitting.

Most families have never been to an orthotist before and aren't sure what they're walking into. Here's the whole process, start to finish, so nothing comes as a surprise.

01

The first appointment

About 45 minutes. I'll ask about your child's history, watch them sit, stand or walk, and take measurements. Bring any letters, X-rays or reports you have — and your child's usual shoes.

Scanning or casting

A 3D scan takes seconds and nothing touches your child. Where scanning isn't practical, we take a plaster cast instead. Babies can stay in your arms for this.

10–20 min

Design and making

Your child's scan is shaped on screen, then the device is manufactured and finished. Nothing is taken off a shelf — every device is built to that one scan.

1–3 weeks

The fitting

We check the fit against the skin, adjust it, and agree a wear schedule. You'll leave knowing exactly how many hours a day, what to look for, and who to call.

Follow-up

Children grow, so devices are reviewed and adjusted. Helmets are checked every few weeks; braces every few months.

Questions parents ask

The things
everyone asks.

These come up in almost every first appointment, so they may as well be answered before you get there.

Does it hurt?

No. A well-made orthosis shouldn't be painful — pressure, yes, in specific places, because that's how it works, but not pain. Redness that fades within about twenty minutes of taking the device off is normal. Redness that stays, or any broken skin, means stop wearing it and call me.

How many hours a day does it have to be worn?

It depends entirely on the device and the condition. Cranial helmets are usually worn most of the day and night. Scoliosis braces are prescribed for a set number of hours based on the curve and your specialist's plan. Walking orthoses are generally worn whenever your child is up and about.

Wearing time is agreed at the fitting, not guessed at — and it is the single thing that most affects whether treatment works.

Can my child wear it at school?

Usually yes, and for scoliosis braces and walking orthoses that's normally expected. Braces fit under clothing and most classmates never notice. I'm happy to write a short note for the school explaining what the device is and what your child can and can't do.

What about the summer here?

Heat is a real issue in the UAE and it's the most common reason children stop wearing a device. Ventilation holes, breathable liners and a cotton layer underneath all help. If your child is struggling in summer, tell me — there are usually adjustments that make it manageable. Quietly giving up is the worst outcome.

Will insurance cover it?

It varies a great deal between insurers and between policies. Many UAE policies cover orthotic devices with a doctor's prescription and prior approval; some exclude them entirely; some cover part of the cost.

Bring your policy details to the first appointment. The clinic can check your coverage and get approval before anything is made, so you're not left with a surprise.

Do we need a referral from a doctor?

Most families come through a paediatrician, orthopaedic surgeon or physiotherapist, and for most conditions that assessment is a necessary part of getting it right. Insurers generally ask for it too.

If you don't have one yet, start with your paediatrician. You're also welcome to message me first with what you're seeing — I can tell you whether it's the sort of thing worth having looked at.

What if my child refuses to wear it?

It happens, and it's worth saying out loud rather than hiding. Usually there's a reason: it rubs somewhere, it's hot, it's uncomfortable at night, or an older child is self-conscious about it. Almost all of those have a fix.

Come back in rather than pushing through. A device that gets worn imperfectly but consistently beats a perfect one sitting in a cupboard.

How often will we need to come back?

Cranial helmets need checking every two to four weeks, because babies' heads change quickly. Scoliosis braces are typically reviewed every three to six months during growth. Walking orthoses are usually reviewed every six months, or sooner if your child has outgrown them.

I trained as an orthotist-prosthetist in Paris, where the job is taught as a craft: you measure the patient, you shape the device, and you're the one who fits it. Before moving here I ran the orthotics and prosthetics department at Fondation Ellen Poidatz, working alongside orthopaedic surgeons and physiotherapists.

Since 2022 I've been in Dubai, seeing patients at hospitals and rehabilitation centres across Dubai, Abu Dhabi and Al Ain. Most of my week is children — scoliosis bracing, cranial remoulding, walking orthoses for children with cerebral palsy, and seating for children who can't sit unsupported.

The same person assesses your child, designs the device and fits it. Nothing gets handed off halfway.

نادرًا ما يأتي الأهل ومعهم تشخيص. يأتون واصفين ما يرونه. في ما يلي ما تعنيه تلك الأوصاف عادةً، وما يمكن عمله في كل حالة.

لا شيء هنا تشخيص. كل طفل مختلف، والطريقة الوحيدة للتأكد هي التقييم.

جهاز تقويم الكاحل والقدم · أي عمر

قدم طفلي تُجرّ أو تلتفّ إلى الداخل عند المشي

يثبّت جهاز تقويم الكاحل والقدم وضعية القدم في كل خطوة، فلا تضطر الساق إلى الالتفاف لتتجاوز الأرض. شائع بعد السكتة الدماغية، وفي الشلل الدماغي، وعند الأطفال الذين لم يستقرّ نمط مشيهم بعد.

جهاز تقويم الركبة والكاحل والقدم · أي عمر

ركبة طفلي تنثني فجأة تحته

عندما تعجز الركبة عن تحمّل الوزن وحدها، يتولّى جهاز يغطي الساق كاملة هذه المهمة. المفاصل إمّا تُقفل لتوفير الثبات أو تتحرك بحرية، بحسب قدرة طفلك على التحكم.

مشدّ الجنف · عادةً 8–16 سنة

ظهرها يبدو غير متساوٍ عندما تنحني إلى الأمام

كتف أعلى من الآخر، بروز في الأضلاع من جهة، خصر غير متناظر. إذا أكّد الأخصائي وجود انحناء، يُستخدم مشدّ يُلبس خلال سنوات النمو لإيقاف تفاقمه.

خوذة تصحيح شكل الرأس · من عمر 4 أشهر

رأس رضيعي يبدو مسطّحًا من جهة واحدة

حالة شائعة جدًا وقابلة للتصحيح غالبًا. إذا لم يُجدِ تغيير وضعية النوم بحلول الشهر الرابع أو الخامس، توجّه خوذة مخصّصة نموّ الجمجمة بلطف. لا تسبب ألمًا، ويتوقف الرضّع عن ملاحظتها خلال أيام.

نظام جلوس مخصّص · أي عمر

طفلي لا يستطيع الجلوس دون إسناد

يدعم نظام الجلوس المصبوب الجذع والرأس ليجلس الطفل بأمان — ما يعني عادةً تنفسًا وأكلًا أسهل، ومشاركة فيما يدور حوله.

النعال والأجهزة التقويمية للقدم · أي عمر

طفلي يشتكي من ألم في قدميه بعد المشي

توزّع النعال المخصّصة الضغط على القدم وتدعم القوس. تُستخدم أيضًا لقدم مرضى السكري، حيث تكون حماية الجلد أهم من الراحة.

الأحذية الطبية المخصّصة · أي عمر

الأحذية العادية لا تناسبه

حين يكون شكل القدم مختلفًا، أو إحدى الساقين أقصر، أو يحتاج الجهاز إلى مساحة داخل الحذاء، يُصنع الحذاء على المقاس بدل تعديل حذاء جاهز.

تصحيح شكل الأذن · الأسابيع الأولى من العمر

أذن مولودي مطوية

غضروف الأذن ليّن في الأسابيع الأولى بعد الولادة ويمكن إعادة تشكيله غالبًا دون جراحة. هذه أكثر الحالات حساسية للوقت — يُفضّل خلال الأسابيع الثلاثة الأولى.

لماذا التوقيت مهم

بعض هذه العلاجات
له نافذة زمنية.

ليس كل شيء عاجلًا. لكن بعض هذه العلاجات يعمل عبر توجيه النمو — أي أنه لا ينفع إلا ما دام النمو مستمرًا. إذا كنت تفكّر في الانتظار قليلًا، فهذا هو الجزء الذي يستحق القراءة.

تصحيح شكل الأذن

أول 3 أسابيع

يكون الغضروف في أليَن حالاته مباشرة بعد الولادة. بعد نحو ستة أسابيع يتصلّب، فيصبح التصحيح دون جراحة أصعب بكثير أو غير ممكن.

خوذة الرأس

من 4 إلى 6 أشهر

تنمو الجمجمة بأسرع وتيرة في السنة الأولى، والخوذة تعمل بتوجيه هذا النمو. البدء بين الشهر الرابع والسادس هو المعتاد، ونادرًا ما يُجدي البدء بعد الشهر الرابع عشر.

مشدّ الجنف

خلال فترة النمو

يمكن للمشدّ أن يؤثر في الانحناء ما دام العمود الفقري ينمو. بعد اكتمال النمو لا يعود المشدّ يغيّر الانحناء، وإن استُخدم أحيانًا للراحة.

أجهزة المشي

حالما تلاحظ الأمر

لا توجد نافذة محددة هنا. لكن كلما طال بقاء نمط مشي غير طبيعي دون دعم، زاد ما يعوّضه الورك والركبة والظهر.

إذا لم تكن متأكدًا هل الوقت مبكر جدًا أم متأخر — اسأل. الوقت المبكر لم يكن مشكلة لأحد قط.

معظم العائلات لم تزر أخصائي تقويم من قبل ولا تعرف ما الذي ينتظرها. إليك العملية كاملة من البداية إلى النهاية، حتى لا تكون هناك مفاجآت.

الزيارة الأولى

نحو 45 دقيقة. سأسأل عن تاريخ طفلك الصحي، وأراقبه وهو يجلس أو يقف أو يمشي، وآخذ القياسات. أحضر أي تقارير أو صور أشعة لديك — وحذاء طفلك المعتاد.

المسح أو القالب

المسح ثلاثي الأبعاد يستغرق ثوانٍ ولا شيء يلامس طفلك. وحين لا يكون المسح عمليًا نأخذ قالبًا جبسيًا بدلًا منه. يمكن للرضّع البقاء بين ذراعيك خلال ذلك.

التصميم والتصنيع

يُشكَّل مسح طفلك على الشاشة، ثم يُصنّع الجهاز ويُنهى. لا شيء يؤخذ جاهزًا — كل جهاز مبني على ذلك المسح وحده.

التركيب

نتحقق من ملاءمة الجهاز للجلد، ونعدّله، ونتفق على جدول الارتداء. ستغادر وأنت تعرف عدد الساعات يوميًا، وما الذي تنتبه له، وبمن تتصل.

المتابعة

الأطفال ينمون، لذا تُراجَع الأجهزة وتُعدَّل. الخوذات تُفحص كل بضعة أسابيع، والمشدّات كل بضعة أشهر.

الأسئلة التي
يسألها الجميع.

هذه الأسئلة تتكرر في كل زيارة أولى تقريبًا، فمن الأفضل الإجابة عنها قبل أن تصل.

هل يسبب ألمًا؟

لا. الجهاز التقويمي المصنوع جيدًا لا ينبغي أن يكون مؤلمًا — ضغط في مواضع محددة نعم، لأن هذه هي طريقة عمله، لكن ليس ألمًا. الاحمرار الذي يزول خلال نحو عشرين دقيقة من خلع الجهاز طبيعي. أما الاحمرار الذي يبقى، أو أي تشقق في الجلد، فيعني التوقف عن الارتداء والاتصال بي.

كم ساعة يوميًا يجب ارتداؤه؟

يعتمد كليًا على الجهاز والحالة. خوذات الرأس تُرتدى عادةً معظم اليوم والليلة. مشدّات الجنف توصف لعدد ساعات محدد بحسب الانحناء وخطة الأخصائي. أجهزة المشي تُرتدى عمومًا كلما كان الطفل مستيقظًا ومتحركًا.

يُتفق على مدة الارتداء عند التركيب، لا بالتخمين — وهي العامل الأكبر في نجاح العلاج من عدمه.

هل يمكن لطفلي ارتداؤه في المدرسة؟

عادةً نعم، وبالنسبة لمشدّات الجنف وأجهزة المشي هذا هو المتوقع. المشدّات تُلبس تحت الملابس ولا يلاحظها معظم الزملاء. ويسعدني كتابة ملاحظة قصيرة للمدرسة تشرح ما هو الجهاز وما يستطيع طفلك فعله وما لا يستطيع.

وماذا عن الصيف هنا؟

الحرارة مشكلة حقيقية في الإمارات، وهي السبب الأكثر شيوعًا لتوقف الأطفال عن ارتداء الجهاز. فتحات التهوية والبطانات القابلة للتنفس وطبقة قطنية تحت الجهاز كلها تساعد. إذا كان طفلك يعاني في الصيف، أخبرني — هناك دائمًا تعديلات تجعل الأمر محتملًا. الاستسلام بصمت هو أسوأ النتائج.

هل يغطيه التأمين؟

يختلف الأمر كثيرًا بين شركات التأمين وبين الوثائق. كثير من وثائق الإمارات تغطي الأجهزة التقويمية بوصفة طبية وموافقة مسبقة؛ بعضها يستثنيها تمامًا؛ وبعضها يغطي جزءًا من التكلفة.

أحضر تفاصيل وثيقتك إلى الزيارة الأولى. تستطيع العيادة التحقق من التغطية والحصول على الموافقة قبل تصنيع أي شيء، حتى لا تفاجأ لاحقًا.

هل نحتاج إلى إحالة من طبيب؟

معظم العائلات تأتي عبر طبيب أطفال أو جراح عظام أو أخصائي علاج طبيعي، وفي معظم الحالات يكون هذا التقييم جزءًا ضروريًا من العلاج الصحيح. وشركات التأمين تطلبه عادةً أيضًا.

إذا لم يكن لديك إحالة بعد، ابدأ بطبيب الأطفال. ويمكنك أيضًا مراسلتي أولًا بما تلاحظه — سأخبرك إن كان الأمر يستحق الفحص.

ماذا لو رفض طفلي ارتداءه؟

يحدث هذا، ومن الأفضل قوله بصراحة بدل إخفائه. عادةً هناك سبب: احتكاك في موضع ما، أو حرارة، أو انزعاج ليلًا، أو خجل عند الأطفال الأكبر سنًا. ولكلٍّ من هذه حلّ تقريبًا.

عُد إليّ بدل الضغط على الطفل. جهاز يُرتدى بانتظام رغم النقص أفضل من جهاز مثالي في الخزانة.

كم مرة سنحتاج إلى العودة؟

خوذات الرأس تحتاج فحصًا كل أسبوعين إلى أربعة أسابيع، لأن رؤوس الرضّع تتغير بسرعة. مشدّات الجنف تُراجع عادةً كل ثلاثة إلى ستة أشهر خلال النمو. أجهزة المشي تُراجع كل ستة أشهر، أو أبكر إذا كبر طفلك عليها.

تدرّبت كأخصائي تقويم وأطراف صناعية في باريس، حيث تُدرَّس المهنة كحرفة: تقيس المريض، وتشكّل الجهاز، وأنت من يركّبه. قبل انتقالي إلى هنا ترأست قسم التقويم والأطراف الصناعية في مؤسسة إيلين بوادا، بالعمل إلى جانب جراحي العظام وأخصائيي العلاج الطبيعي.

منذ 2022 وأنا في دبي، أستقبل المرضى في مستشفيات ومراكز تأهيل في دبي وأبوظبي والعين. معظم أسبوعي مع الأطفال — مشدّات الجنف، وخوذات تصحيح شكل الرأس، وأجهزة المشي لأطفال الشلل الدماغي، وأنظمة الجلوس لمن لا يستطيع الجلوس دون إسناد.

الشخص نفسه يقيّم طفلك، ويصمم الجهاز، ويركّبه. لا شيء يُسلَّم لطرف آخر في المنتصف.
